How Is The Female To Male Sex Change Operation Performed?

A female-to-male sex change operation can involve a range of treatments and procedures in order to obtain the desired results. They may be executed sequentially or at different times, as mentioned below.

Hormone Therapy

In most all transgender males, the surgeon first prescribes hormone therapy for boosting testosterone levels. Testosterone triggers various secondary sex characteristics of the desired sex and causes the clitoris to grow in size. The clitoris and the penis head are functionally similar, and testosterone helps enlarge the clitoris.

Hysterectomy

Hysterectomy is performed to surgically remove the uterus along with the cervix to stop mestruation. These parts aren’t a part of the male genitilia, and hence, are needed to be removed to stop certain hormone responses as well.

In a partial hysterectomy, only the uterus part is removed, while a total hysterectomy is performed to remove cervix as well.

Oophorectomy

A bilateral salpingo-oophorectomy, or BSO, is performed to remove the right and left ovaries along with the fallopian tubes.

Metoidioplasty

Before this procedure, a vaginectomy may also be performed to carefully remove the vaginal tube, cheeks and libia (lips).

A Centurion procedure may also be performed to reposition round ligaments under the clitoris to increase the girth of the neopenis.

The vaginal tube, cheeks and libia removed during the vaginectomy will be wrapped around the urethra to form its girth. All parts will be sutured to allow them to merge as they heal.

Phalloplasty

The neopenis created during metoidioplasty won’t have enough length or girth to look like a natural penis. So, to ensure a full female-to-male transition physically and mentally, trans males can choose to have a phalloplasty to futher enhance its appearance.

Here, the surgeon take adequate skin grafts from the arm, thigh, back, or abdomen to increase the thickness of the neopenis, and may perform a vaginectomy to allow urination through the penis head.

Scrotoplasty

This is often the final procedure during a female-to-male sex change operation, where a trans man can opt to have implanted testicles to suit the neopenis.

If you opt for this procedure, the skin from the libia or the other vaginal lips is used to create the scrotum sack, and two slicone implants resembling testicales are placed inside.
